This non-discrimination policy covers applications and intakes, access, and treatment in HOM’s programs and activities. It also covers counselors, administrative staff, managerial staff, and volunteers in their employment.

 

Information on the Application Process

To apply to the House of Metamorphosis, please note the following requirements:

  • You have to be 18 years or older
  • Applications are done in-person
  • An intake counselor will guide and assist you
  • Call us Monday through Friday from 8:00 AM to 5:00 PM at to make an appointment
  • If you like, you can walk in Monday through Friday, 8:00 AM to 4:00 PM for an assessment
  • Bring a picture ID (such as a California ID or Driver’s License) and a Social Security Card
  • Medi-Cal/Medi-Care card, Insurance Card
  • If your ID or Social Security Card was lost or stolen, you can bring the receipt for your renewal (for example, from the DMV)

 

The intake application covers a variety of information about you that helps us get to know you. Please be prepared to provide the following information:

  • Demographic and Identifying Information
  • Social history
  • Drug and alcohol history
  • Mental health history
  • Legal status
  • Financial information
  • Vocational achievements
  • Education
  • Medical history
  • Previous treatment episodes
  • Medications (including psychotropic medications)
  • Self assessment

Only upon completion of the intake process and signing of the HOM contractual agreement will a client be admitted into treatment. We make every attempt to provide treatment to all potential clients. If it is in the best interest of the applicant, we will provide referrals to other programs and facilities.
